Before you send
What should I include with the referral?
The patient's name and contact information, the reason for referral, and any current records such as radiographs, CBCT imaging, charting, or photos. Records can be emailed to [email protected] or our team will coordinate the transfer with your office. Missing records should never delay a referral; imaging can be obtained here.
What if the case is urgent?
Call the practice directly at (202) 244-2101 and tell the team the referral is urgent. A fractured anterior tooth, a failing restoration in the esthetic zone, or a patient in discomfort will be prioritized.
Does the patient need to do anything?
No. Once you submit the referral, our concierge contacts the patient directly and handles scheduling, records, and logistics. If the patient prefers to reach out themselves, they can request an appointment through the website or call the practice and mention your referral.
