When you commit to full mouth reconstruction, you are committing to a clinical sequence that begins with diagnosis and ends with long-term maintenance. Understanding what happens in each phase helps you anticipate what to expect and why each step matters.

Diagnostic Sequencing: Building the Clinical Foundation

Your reconstruction begins with comprehensive diagnostic information. You have a detailed clinical examination where each tooth is assessed for structure, support, and potential. Three-dimensional imaging via CBCT reveals bone density, bone height, and the anatomical constraints that will influence where implants can be placed and how roots behave.

Digital scans capture the precise contours of your existing dentition and tissues. Photographs document your current smile, your lip line, the visibility of your teeth, and the contours of your gums from multiple angles. You have a detailed bite analysis where we record how your teeth contact, where the pressure is concentrated, and whether there are interference patterns that need correction.

You also provide your goals. You describe what is bothering you functionally, what you want your smile to look like, any esthetic priorities, and any practical constraints on timing or scheduling. This information becomes part of the diagnostic database.

All of this information flows into a treatment plan that is written, detailed, and reviewed with you. The plan specifies what will happen, in what sequence, with what timeline, and with what expected costs.

Pre-Surgical Stabilization: Managing Immediate Concerns

Before any surgical phase begins, immediate problems must be stabilized. A tooth with active infection receives endodontic treatment or extraction. A restoration that is failing is replaced with interim work that stabilizes it until it can be permanently restored or extracted.

This stabilization phase ensures that you are comfortable and functional before the surgical work that follows. It typically takes two to four weeks. By the end of it, there are no active infections, no failing restorations that are interfering with daily life, and no acute pain.

Surgical Preparation: Extraction, Grafting, and Implant Placement

The surgical phase is tailored to your specific clinical needs. If teeth require extraction, extractions happen in this phase. Teeth with good prognosis and good bone support are retained. Teeth that cannot be reliably restored or that would compromise the reconstruction are extracted.

Where bone is inadequate for implant placement, bone grafting or ridge augmentation happens in this phase. A sinus lift may be required to create sufficient bone height in the upper jaw posterior region. These bone procedures take time to heal, which is built into your timeline.

Implants are placed at implant sites according to the prosthetic plan. They are positioned to support the eventual restorations, not simply placed where bone is available. The surgical phase is typically completed in one or two appointments, depending on complexity.

During the surgical phase, you wear interim restorations that maintain your function and appearance. You do not go without teeth or restorations while healing is occurring.

Integration Monitoring: The Waiting Period

After implant placement, there is a healing period during which the bone bonds with the implant. This integration period is determined by biology, not by your preference for speed. Lower jaw integration typically takes 4 to 6 months. Upper jaw integration takes 5 to 9 months. Grafted sites may require additional time.

You return for periodic appointments to monitor healing. These are brief, non-invasive check-ups. You wear interim restorations during this entire period, so you continue to function normally. The tempo of appointments is slow during this phase, which is convenient for patients with full professional schedules.

Restorative Fabrication: Design and Try-In

Once integration is confirmed, the restorative phase begins. Detailed impressions or digital scans are captured of the integrated implants and of teeth that will be restored with crowns or bridges.

Our on-site laboratory designs the restorations based on the treatment plan. Abutments are fabricated. Crowns or bridges are designed with attention to fit, function, and esthetics. All of this happens in the laboratory while you continue your normal life between appointments.

A try-in appointment is scheduled where the proposed restorations are inserted temporarily and evaluated. You see how they look. We check the bite, the contact, the spacing. This is your opportunity to provide feedback before anything is irreversible. If adjustments are needed, they are noted, modifications are made in the laboratory, and you return for another try-in.

Once the try-in is approved, the final fabrication details are completed. The restorations are prepared for permanent placement.

Final Delivery: Placement and Stabilization

Final delivery is when the permanent restorations are cemented or attached. For a complex reconstruction, this may happen in multiple appointments, completing one section at a time. For a simpler reconstruction, it may be completed in a single visit.

At final delivery, we verify that all restorations are fitting properly, the bite is stable, and you are comfortable. You leave with your reconstruction complete and your mouth functional.

Maintenance Planning: Long-Term Protection

After reconstruction, ongoing maintenance keeps your restorations healthy and stable. Initial maintenance appointments are every three months for the first year. Afterward, appointments typically space to every six months.

Maintenance includes professional cleaning, bite evaluation, inspection of all restorations, and radiographs to monitor bone levels around implants. You also maintain daily care: brushing, interdental cleaning, and any specific protocols recommended for your case.

The Timeline from Start to Finish

A reconstruction without extractions or implants may proceed to restorations within three to four months. A reconstruction with extraction and implant healing typically requires 10 to 14 months. Complex cases with multiple phases and extended integration may take 18 months or longer.
