The Cosmetic Case Sequence: What Happens at Each Treatment Phase

Understand the cosmetic dentistry treatment sequence from diagnostics through delivery. What happens at each chairside phase.

Cosmetic dentistry follows a predictable sequence of procedures, each serving a specific purpose. Understanding the cosmetic case sequence—what happens at each phase, what the dentist is doing at the chairside, and why each step matters—demystifies treatment and helps patients understand what to expect.

Phase One: Diagnostic Records and Assessment

Before any tooth is touched, comprehensive diagnostic information is gathered. This phase informs all subsequent decision-making.

The process begins with intraoral photographs taken from standardized angles. These images capture current smile aesthetics, tooth color, shape, and alignment. Extraoral photographs show how teeth relate to facial features—essential because cosmetic dentistry must harmonize teeth with the patient’s face rather than imposing a generic smile.

Radiographs (X-rays) are obtained to assess bone levels, identify any internal pathology, and evaluate tooth vitality. These images ensure the foundation is sound before cosmetic work proceeds.

Shade assessment involves measuring current tooth color using standardized shade guides and spectrophotometry. This baseline measurement ensures proposed shade changes are appropriate and achievable.

Bite analysis examines how upper and lower teeth contact during function. Bite mechanics influence whether cosmetic changes will be stable long-term. Patients with problematic bite patterns may require bite correction before or alongside cosmetic work.

This diagnostic phase typically requires one appointment and provides the information guiding all subsequent clinical decisions.

Phase Two: Smile Design and Patient Approval

With diagnostic information in hand, detailed smile design is performed. This phase is collaborative, involving Dr. Marlin and the patient working together to refine the aesthetic plan.

Digital smile design software imports the diagnostic photographs and allows manipulation of tooth parameters: shade, shape, size, alignment, and position. Dr. Marlin shows the patient different design options and asks for feedback. Does the patient prefer slightly wider teeth or narrower? Does the proposed shade feel right? Is the smile width and proportion appropriate for the patient’s face?

Through this iterative process, a final design emerges that reflects both Dr. Marlin’s professional expertise and the patient’s aesthetic preferences. This collaborative approach prevents surprises and ensures the patient genuinely approves the planned aesthetic outcome.

At the end of this phase, the patient signs off on the design. This approved design becomes the roadmap for all subsequent clinical and laboratory work.

Phase Three: Tooth Preparation and Temporary Placement

The preparation appointment is where cosmetic design becomes clinical reality. This is the most involved chairside visit, typically lasting 1 to 3 hours depending on the number of teeth being prepared.

Local anesthesia is administered. Topical numbing is applied first to reduce injection discomfort. The anesthetic solution is delivered slowly and gently. Most patients are surprised how comfortable the injection is despite their initial anxiety.

Once anesthesia is effective, preparation begins. Dr. Marlin carefully shapes the tooth surfaces according to design specifications. For veneers, the front surface is gently prepared, typically removing less than half a millimeter of tooth structure. For crowns, the entire tooth is prepared. Tools are used at moderate speeds with water cooling to minimize heat generation.

The preparation is performed with precision because the quality of preparation directly influences the quality of the final restoration. Sloppy preparation leads to gaps, poorly fitting restorations, or restorations that require excessive adjustment.

As preparation progresses, impressions or digital scans are obtained. These records capture the prepared tooth anatomy in detail, allowing the laboratory to fabricate restorations that fit perfectly.

Temporary restorations are then fabricated and placed. Using the digital design specifications, Dr. Marlin shapes temporary acrylic restorations that approximate the final aesthetic plan. These temporaries are not as refined as permanent restorations but are adequate for protection and function during laboratory fabrication.

By the end of this appointment, the patient is wearing temporaries that represent his anticipated aesthetic improvement. This allows him to experience how his new smile will look and feel before final restorations are placed.

Phase Four: Laboratory Fabrication

Following the preparation appointment, impressions are sent to the laboratory with detailed design specifications. The laboratory becomes part of the treatment team.

The ceramist (laboratory technician) receives the impressions and design information. Using premium porcelain and meticulous hand-craftsmanship, the ceramist fabricates individual restorations. Shade is selected by matching the design shade specifications under calibrated lighting. The ceramist custom-shapes each restoration to achieve the proportions and contours specified in the design.

At Elite Prosthetic Dentistry, Dr. Marlin works with an on-site laboratory, which means direct collaboration between dentist and ceramist. Rather than communicating by email about shade or contour adjustments, Dr. Marlin and the ceramist discuss specifications in person under controlled lighting. This collaboration typically produces superior results because adjustments happen in real-time rather than through multiple rounds of revision.

Laboratory fabrication typically requires two to four weeks depending on restoration complexity and current laboratory workload.

Phase Five: Try-In and Delivery

Once the laboratory completes restorations, the delivery appointment is scheduled. This appointment is the culmination of the entire treatment sequence.

The temporary restorations are carefully removed. The prepared teeth are cleaned and inspected to ensure they remain healthy.

Each permanent restoration is then tried into position. Dr. Marlin checks:

  • Fit: Does the restoration seat completely and smoothly on the prepared tooth? Are margins (junction between restoration and tooth) perfectly adapted with no visible gaps? Does the contact point fit properly?

  • Shade: Does the color match the approved design and adjacent natural teeth? Is the shade consistent across multiple restorations if multiple teeth are being treated?

  • Contour: Does the shape match the design specifications? Do the tooth proportions appear natural and appropriate for the patient’s face?

  • Bite: How do the restorations contact with opposing teeth? Is the contact appropriate and balanced? Would this bite create problems during function?

If everything is perfect, restorations are bonded permanently to prepared teeth using adhesive cement. The bonding process is quick and well-tolerated. Excess cement is carefully removed and the bite is verified one final time.

If minor issues are identified during try-in, adjustments are made before permanent placement. Common adjustments include: shade refinement, contact point adjustment, bite contact modification, or contour refinement. These adjustments are performed while restorations are still removable, ensuring the patient gets exactly what was approved.

The delivery appointment typically lasts 1 to 2 hours. By the end, permanent restorations are bonded and the cosmetic improvement is complete.

Phase Six: Post-Operative Review and Adjustment

A follow-up appointment is scheduled 5 to 10 days after delivery. By this time, the mouth has adjusted to new restorations. Any bite discomfort or edge sensitivity that initially existed typically resolves naturally. However, this follow-up appointment ensures everything is genuinely comfortable.

During this visit, bite contact is verified. If any specific areas feel uncomfortable, Dr. Marlin makes minor adjustments. Edge smoothness is checked—any rough or sharp feeling areas are polished smooth. Overall comfort is verified.

This follow-up typically lasts 15 to 30 minutes. By the end of this appointment, the patient should feel completely comfortable with his restorations.

The Complete Sequence Timeline

For a typical cosmetic case involving multiple veneers or crowns, the complete sequence from initial consultation to final adjustment typically spans 6 to 8 weeks:

  • Week 0: Consultation and diagnostic records
  • Week 1: Smile design appointment
  • Week 2: Preparation and temporary placement
  • Week 3-4: Laboratory fabrication
  • Week 5: Delivery and permanent placement
  • Week 6: Post-operative follow-up

This timeline can be compressed (with expedited laboratory service) or extended (if the patient prefers phased treatment). Dr. Marlin discusses timeline options during consultation.

Understanding the cosmetic case sequence helps patients recognize that cosmetic dentistry is thoughtfully structured, with each phase serving a specific purpose. The sequence ensures quality outcomes while respecting the patient’s time and investment.

Frequently Asked Questions

How much of my tooth structure is removed during cosmetic preparation?

The amount of tooth removal depends on treatment type. Veneers require minimal preparation, typically less than half a millimeter of tooth structure from the front surface. Crowns require more substantial preparation because they cover the entire tooth, but this remains far more conservative than historical approaches. Dr. Marlin always uses the most conservative preparation possible while achieving the desired aesthetic result.

What anesthesia is used during cosmetic treatment?

Local anesthesia (numbing injection) ensures you feel minimal discomfort during tooth preparation. Topical numbing is applied first to minimize injection discomfort. Modern anesthetic solutions are comfortable and effective. Many patients are surprised how comfortable the procedure feels despite the comprehensive nature of the work. No general anesthesia or sedation is required for cosmetic dentistry.

How are temporary restorations fabricated during the preparation appointment?

Temporary restorations are fabricated immediately after tooth preparation. Dr. Marlin uses the digital design specifications to create temporaries that closely approximate your final restorations' shape and shade. These temporaries are not as refined as final restorations but are adequate for function and appearance during the laboratory fabrication phase.

Can I feel the difference between temporary and final restorations when they're placed?

Yes. Temporary restorations are acrylic and feel slightly different than permanent porcelain. They may taste different and feel slightly bulkier. These differences are temporary and disappear the moment your final restorations are placed. Most patients find the transition from temporary to permanent restorations to be the most satisfying moment of treatment.

What happens if the fit isn't perfect when final restorations are delivered?

Excellent laboratory fabrication and careful chairside try-in minimize fit issues, but Dr. Marlin checks fit meticulously before placing any restoration. If a margin isn't perfect or a contact point doesn't seat properly, adjustments are made before permanent placement. Patient comfort and proper fit are non-negotiable before final bonding.
